Pelabresib ‘AML transformation’ cases revealed
A limited amount of new information about an emerging safety issue related to pelabresib, Novartis’ experimental drug for myelofibrosis, was revealed at an update to a Phase 3 clinical trial on Friday afternoon. Fans and critics of the controversial drug each saw and heard something to support their respective cases.

What we now know: Five pelabresib-treated patients in the study rapidly progressed to a “blast phase” of acute myeloid leukemia, a more aggressive and deadlier type of blood cancer, compared to one patient in the study’s control arm. Later, one of the five pelabresib patients was re-diagnosed with lymphoma, so the “AML transformation” case count was 4:1 — a rate of 2.4% in the pelabresib arm compared to 0.5% in the control arm.

In April, I reported that investigators in the study were examining reports of six cases of AML transformation in the pelabresib study. AML that arises from myelofibrosis occurs infrequently, but is resistant to most treatments and is associated with a poor prognosis, including median overall survival measured in months.

Progression from myelofibrosis to the more serious “blast phase” of AML is “unfortunately, a part of the natural history of the disease,” said Raajit Ranpal of Memorial Sloan Kettering Cancer Center and an investigator in the pelabresib study who presented the update on Friday. He is also a paid consultant to Novartis. The data he presented were also prepared with the assistance of a group funded by MorphoSys, which owned pelabresib before the company was sold to Novartis.

Ranpal said very little else about the cases, except noting in his slides that “leukemic transformations continue to be followed up.”

The ASCO session included a discussion of data by Pankit Vachhani from the University of Alabama at Birmingham. Vacchani was not involved in the pelabresib study but he has received consulting fees from Novartis and MorphoSys.

“I’m not saying it’s nothing,” said Vachhani, referring to the AML transformation cases. “We should keep an eye on it and follow it more closely over time.”

Novartis did not respond to a request for comment.

Nein, Novartis hat super Daten zu Scemblix in 1L CML vorgelegt, dem Nach-Nachfolger von Gleevec. Bessere Wirkung (MMR) bei weniger Nebenwirkungen im Vergleich zu den Competitors.

Interessant fand ich noch, dass einige institutionelle offensichtlich nicht alles im ersten Lauf angedient haben und noch einen Restbestand gemeldet haben.

Sehr schwaches Bild, die ASCO-Präsentation. Fast nur 24 week data... ein komischer Plot, der länger geht, aber mMn wenig aussagt (time to splenic response). Am TSS war der Fatigue-Score schuld, ohne den ist p etwas kleiner, aber immer noch nicht signifikant... "leucemic transformation continues to be followed-up", sonst nichts dazu. Gebt bei Twitter Pelabresib und Manifest-2 ein, sortieren nach latest.
